Northwest Florida Health Council (Dist. 1)

Northwest Florida & Big Bend Health Councils
The Northwest Florida Health Council is a private, non-profit health planning organization serving Escambia, Okaloosa, Santa Rosa and Walton counties since 1969.  The Council is governed by twenty-one concerned citizens who are appointed by the fourteen county commissions of the service area. 

The Council members are health leaders within their communities.  They either provide or purchase health care for large segments of the community, or they are interested, active, and concerned consumers.  The Council shares a staff of health planning professionals with the Big Bend Health Council, as well as a central office located in Panama City.

Funding Announcement 

Big Bend Health Council in collaboration with Health Planning Council of SW Florida and WellFlorida Council, Inc., are pleased to announce that the following agencies have been granted funding for the development or expansion of Diabetes Self-Management Education programs for November 2021 - June 2022:

 

  • Ascension Sacred Heart Gulf
  • Rural Health Care, Inc., Aza Health
  • Bay Street Pharmacy
  • Big Bend Area Health Education Center
  • CINET, Inc.
  • Core Health Partners
  • Family Health Centers of Southwest Florida, Inc.
  • Flagler County Free Clinic
  • Heart of Florida Health Center
  • PanCare of Florida, Inc.
  • Premier Community Healthcare Group,Inc.
  • The Way Free Medical Clinics, Inc.


We received a record number of applications this year, and we're pleased to see so much interest in the program. We carefully reviewed all the applications and it was very difficult to select the applications to fund with so many great programs across the state. We will be issuing another appliaction for funding in the summer of 2022, and hope that many of the agencies who were not funded this round, will reapply at that time. In the meantime, we will be sharing educational materials related to diabetes self-management education throughout the year. Thank you all for all you do to help improve the health of the people of Florida.
